Short answer: When you stop Wegovy (semaglutide), your appetite usually returns, and studies show many people regain a significant part of the weight over the following year. Improvements in blood sugar and blood pressure can also partly fade. Why appetite comes back. Wegovy works by acting on appetite signals. When it leaves your system, those signals return to their prior state, so hunger and food “noise” often come back. That’s a drug effect, not a failure of willpower.
What the studies show about weight. In research where people stopped semaglutide, a large share of the lost weight returned over about a year. In the STEP 1 trial extension (Wilding et al., 2022), participants regained on average about two-thirds of the weight they had lost within a year of stopping the drug and lifestyle program. Individual results vary a lot, and habits, support, and other treatments matter.
What can fade along with the weight:
- Blood sugar and blood pressure improvements can partly reverse as weight returns.
- Kidney-friendly changes — like lower urine protein and reduced hyperfiltration — depend on keeping the weight off and the metabolic gains intact. If those reverse, the kidney benefit can fade too.
- This is why obesity is increasingly treated as a long-term condition — similar to blood pressure — rather than something you fix once.
Is stopping dangerous for the kidneys? Stopping Wegovy is not known to cause a sudden kidney problem by itself. The kidney concern is slower and indirect: if weight, blood pressure, and blood sugar climb back up, the pressures that stress the kidney can return over time.
How to stop thoughtfully (with your doctor):
- Have a plan before you stop — for diet, activity (including strength training to protect muscle), and follow-up.
- Ask about options if the reason is cost, side effects, or supply — sometimes a different dose, drug, or a maintenance approach is possible.
- Keep monitoring your weight, blood pressure, blood sugar, and — if you have CKD — your kidney labs, so changes are caught early.
- Don’t stop abruptly without telling your prescriber, especially if you take it for diabetes.
Bottom line. Stopping Wegovy commonly brings back appetite and some weight, and can let earlier improvements fade — so treat stopping as a planned transition, not just an on/off switch.
